1. 法令等の遵守

株式会社Fast Fitness Japan（以下「当社」といいます。）は、個人情報を取り扱うにあたって、「個人情報の保護に関する法律」をはじめとする個人情報の保護に関する法令、ガイドラインおよび本プライバシーポリシーを遵守いたします。本プライバシーポリシーは、お客様が当社が提供するサービス及び商品販売（以下併せて「当社サービス等」といいます。）をご利用される場合に適用されます。

エニタイムフィットネスをはじめとする当社サービス等をご利用になる場合には、まず本プライバシーポリシーをお読みください。お客様が当社サービス等をご利用される場合、本プライバシーポリシーに同意したものとみなします。

2. 個人情報の定義

本プライバシーポリシーにおける「個人情報」とは、生存する個人に関する情報であって、次のいずれかに該当するものをいいます。

4. 個人情報の取得 当社がお客様より取得する個人情報は、法令で定められている事項のほか、以下のとおりです。

  1. お客様の氏名、年齢、性別、生年月日、住所、電話番号、勤務先、通学先、緊急連絡先、家族構成、メールアドレス、顔写真その他お客様に関する情報
  1. 決済およびその方法に関する情報（金融機関口座番号、クレジット番号および電子マネーに関する情報等を含む。）
  1. 各店舗の入館履歴、施設内のセキュリティカメラ映像などの情報
  1. お客様より提示を受けた運転免許証、パスポート、健康保険証等、住民票その他お客様の身分を証明する書類の記載情報

5. 第三者提供の制限

当社は、以下の各号に該当する場合を除き、当社が取り扱うお客様の個人情報をあらかじめお客様のご承諾を得ずに第三者に提供または開示いたしません。

  1. 人の生命、身体または財産の保護のために必要がある場合であって、お客様の同意を得ることが困難であるとき

7. 個人情報の委託

当社は、業務を円滑に進めお客様により良いサービスを提供するため、「3. 個人情報の利用目的」に掲げる利用目的の達成に必要な範囲内において、お客様管理システム提供会社または決済業務委託会社等に対し、お客様の個人情報を委託する場合があります。

なお、当社は、決済業務委託会社に対して、お客様の各店舗利用料金等のクレジットカード決済業務を委託する場合があります。この場合、決済業務委託会社からお客様に対し個人情報の提供を求めることがあります。

8. 個人情報の管理・保護について

当社は、個人情報を取り扱う際には、管理責任者を置き、お客様からの届出等を基に情報の正確性を保ち、適切かつ厳重な管理を行うとともに、外部への流出防止に努めます。また、外部からの不正アクセスまたは紛失、破壊、改ざん等の危険に対しては、適切かつ合理的なレベルの組織的・物理的・人的・技術的な安全対策を実施し、個人情報の保護に努めます。当社は、個人情報の利用目的が達成された場合は、遅滞なく個人情報を廃棄・消去いたします。

9. 安全管理措置

当社が取り扱う個人情報の安全管理のため、必要かつ適切な措置として以下の各措置を講じます。

（組織的安全管理措置）

  • 社内各部門に個人情報管理を担当する責任者を指名する等、個人情報を安全に管理するための組織体制の整備に努めています。
  • お客様の個人情報への不正アクセス･個人情報の紛失･改ざん･漏洩等を防止するための取り扱いルール、予防策、モニタリングおよび安全対策を講じます。
  • 万が一にも個人情報の漏洩があるときは、責任者に直ちに報告される組織体制によって、法令の定めるとおりに個人情報委員会（当局）への報告とお客様ご本人への通知を行い、迅速な対応と収拾を実現します。
  • 以上の各措置は、定期的に点検して見直しを適切に行います。

（人的安全管理措置）

  • 全ての従業員に対し、個人情報保護についての必要な研修などを行って周知徹底を図るとともに、個人情報の安全管理が図られるよう必要かつ適切な監督を行います。
  • 個人情報の秘密保持義務は、就業規則に規定し、従業員に徹底させています。

（物理的安全管理措置）

  • 個人情報を取り扱う部署の部屋には、取り扱う者以外の者が安易に入室できない体制です。
  • 個人情報を記録する媒体、書類などは社内で施錠できるキャビネットに保管するなどし、持ち運ぶ場合はパスワードを設定し、施錠した鞄等に入れるなど盗難防止措置をしています。
  • 個人情報を削除したり個人情報を記録する機器、媒体を廃棄するには、責任者が確認します。

（技術的安全管理措置）

  • 個人情報を取り扱う機器および従業員を特定し、正当なアクセス権を有する従業員を機器のユーザーアカウント制御機能で識別しています。
  • 個人情報を取り扱う機器のオペレーティングシステムを最新にし、最新のセキュリテイソフトの導入に努めています。
  • 個人情報を含むファイルをメール等で送信する場合はパスワードの設定や暗号化処理を行います。

10. 個人情報の開示･内容の訂正・追加または削除・利用の停止・消去 当社がお預かりするお客様の個人情報または第三者提供記録に関して、お客様が開示等の請求（利用目的の通知、個人情報または第三者提供記録の開示、訂正、追加、削除、利用の停止、第三者提供の停止の請求）をご希望される場合には、下記「14. お問い合わせ窓口」の電話番号よりお問い合わせください。お客様ご本人から、個人情報の確認・訂正等のお求めがあった場合には、当社は必要に応じて遅滞なく調査を行い、結果に基づき合理的かつ必要な範囲内において速やかに対応させていただきます。 なお、各種手続きに際しては、ご本人である事を確認させていただく場合がございます。また手数料をいただく場合がございますので、その内容につきましても下記「14. お問い合わせ先」の電話番号よりお問い合わせください。  11. 委託先の監督 本プライバシーポリシーに基づいて個人情報を第三者に委託する場合、当社は、委託先に対し個人情報を厳重に管理することを義務付け監督いたします。  12. 変更 当社は、利用目的が変更前と関連性を有すると合理的に認められる場合に限り、個人情報の利用目的を変更するものとします。利用目的の変更を行った場合、変更後の目的について、当社所定の方法により、通知または公表するものとします。 また、当社は本プライバシーポリシーを変更する場合があり、この場合、当ページへの掲載をもって変更とさせていただきます。  13. 免責事項 当社サービス等のウェブサイトには、他社ウェブサイトへのリンクが含まれていますが、当社は当該他社ウェブサイトにおける個人情報の保護などに関する内容については責任を負いません。
